Case Management is a mechanism for assuring that individuals needs is met by coordinating and linking the components of a service. The goal of case management is to provide effective and efficient service to the clients.
During my research I viewed a number of clientele and special population. There were a variety of choices to choose from. My area of interest varies from the abused children, foster care, troubled youth, and the elderly. I chose the elderly population. The elderly population has rapidly increased in the last few years. There is a great need for case managers, social worker and advocates for the elderly. Case managers for the elderly clients help client’s makes their own choices regarding long-term care and to receive services in the home and the community.
This population was chosen because from the age of fourteen years old I have been assisting the elderly in my family and neighboring communities. It has become a special passion of mines. The elderly population needs assistance when seeking services. There are so many resources available; it can become confusing choosing the right service that really meets their immediate needs. There are so many questions to be asked, paperwork to be filled out, along with limitations to service available. The elderly needs and individual with knowledge and expertise to assist them. Seniors can be deprived of benefits that would help them in which they are entitled to. If I could help reduce some of the complexity and ease some of the tension of old age, I will be there to assist.
